By completing this form, I consent to be interviewed, audio recorded, photographed, or videotaped by representatives of the Freedom From Religion Foundation (FFRF) for purposes of publication or broadcast (in all forms of media).

I agree that such interviews, audio recordings, quotes, photographs, or video, and any reproductions of same in any form, are the property of FFRF, and I relinquish any present or future claim for reimbursement for the reproduction of my name, likeness, or statements.

I acknowledge the risks inherent in my name, likeness, or statements being publicized and hereby release FFRF, its affiliates, and its representatives from all claims, demands, costs, and liability that may arise from the use of these interviews or recordings as described above.
